Steps Individuals Must Take To Apply For Services:

  • Call Pete via self referrals, or advocates, family members
  • Obtain referrals to Agency from Resource Coordination
  • Arrangements for a face to face interview
  • A draft of a service plan is submitted to the individual or advocate
  • Review for final approval
