Mission Statement

The mission of the Somerset Community Services is to provide quality supports that are individually tailored to address the specific needs and personal preferences of the individual. Thus, empowering him/her to make informed choices, foster personal growth and become productive members of their community.

Unique or Distinctive Qualities

Somerset Community Services, Inc. provides many unique activities for the individuals served. Just a few of the many activities done annually include: a Fall Formal, Talent Show, Cook Out at Shad landing, Thanksgiving Dinner and a Christmas party with Santa Claus and presents for all the individuals donated by the Foundation of Somerset Community Services.

Somerset Community Services, Inc. utilizes the "Ask Me" results to tailor the goals and outcomes of the Quality Assurance Plan to address any areas of deficiency in consumer satisfaction.

Describe your quality assurance practices, including any agency accreditations.

The Quality Assurance practices at Somerset Community Services, Inc. include five areas of concentration that are felt to be imperative to address in order to provide unique quality services. The areas include Health and Safety, Employee Performance, Individual Fundamental Rights, Behavior Management Programs and Individual Plans. The Standing Committee will meet quarterly in order to thoroughly review and analyze data in the above areas. After analysis, the Standing Committee will be responsible for submitting a report listing recommendation on a systemic and individual level to the Assistant Director of programs, Director of Programs, Executive Director and Board of Directors who will be responsible for implementing the needed changes.

Somerset Community Services, Inc. remains committed to assisting individuals with disabilities in realizing their greatest potential and becoming productive members of their communities by tailoring supports to address the individuals specific needs. Somerste Community Services' Inc. currently provides Day Habilitation, Supported Employment, Community Supported Living Arrangements, Family and Individual Support Services and Residential Services. Somerset Community Services is always receptive to providing new and innovative support services to assist an individual in becoming a productive member of Society.

Steps Individuals Must Take to Apply for Services

Individuals must contact a developmental Disabilities Administration (DDA) Resource Coordinator through the Health Department to apply for services.
