Mission Statement
Center for Social Change's mission is to provide services of the highest quality for people with cognitive and developmental disabilities that optimize each individual's independence and capabilities, ensure self-determination and rights; and with partnerships in the community, enhance their opportunities to live healthy, safe and valued lives.
Unique or Distinctive Qualities
Center for Social Change (CSC) has a history of providing residential services to behaviorally challenging and/or medically fragile individuals. Teams that oversee the homes include Program Managers, Registered Nurses, and other support staff that have experience in providing services to behaviorally challenging and/or medically fragile individuals.
CSC's Supported Employment Program not only assists individuals in locating competitive employment in the community, but has initiated business ventures that will enable individuals to own their own business or to participate in projects that will offer competitive wages. The Supported Employment Program allows individuals to partake of a varied schedule during the week that can include but not be limited to enclaves, volunteer work, entrepreneurial ventures and on-site projects.
CSC opened an adult medical day care facility in May 2007 to assist adults presently in its care, as well as other individuals with disabilities. With a capacity for 100 participants, the service will include supervision in a safe environment, transportation, case management, meals, group activities, recreation, exercise and nursing care.
CSC also opened a Therapeutic Integration Center for Children with autism, first of its kind in Howard County, during the same time to help children with autism to integrate into community.

CSC is reviewing the questionnaires used by CSC's Quality Survey in order to assure that it is measuring the quality of life components (i.e., physical well-being, emotional well-being, interpersonal relationships, social inclusion, personal development, self-determination and rights) as outlined in the Ask Me Survey. CSC considers the Ask Me Survey as a reference model by which to improve its services to individuals with developmental disabilities.

Describe your quality assurance practices, including any agency accreditations.
CSC understands that individuals served in Maryland enjoy many opportunities and choices in their day-to-day life. Due to these choices and opportunities, person-centered services and supports make the delivery of quality services more and more challenging. The quality assurance practices initiated by CSC focus on: understanding that changing expectations are to be expected which result in challenges to quality enhancement; addressing health and safety issues including investigations involving complaints, abuse and/or neglect; fostering the design of control measures that track outcomes and note areas for service improvement; evaluating these control measures and initiating proactive strategies to address improvement; the management of staff recruitment, retention, training, and professional development; and staying current with developments affecting our profession and services, and taking appropriate steps to change, revise, update and/or address policy directions.
Since 1994, CSC has consistently received more than satisfactory licensing reviews by the State. It is in the process of seeking the Maryland Association of Non-Profits' (MANO) Seal of Excellence.
How Does Your Agency Support People In Becoming Active Members Of Their Community?
Center for Social Change's individuals participate in neighborhood associations, recreational leagues, places of worship, community social activities as well as volunteer opportunities. Center for Social Change maintains a full activity schedule with the assistance of an active volunteer program which is administered by a Volunteer Coordinator.
It has been CSC's experience that individuals that are active members in their community, with the proper supports and supervision, enjoy an enhanced quality of life.
Steps Individuals Must Take to Apply for Services
- Establish eligibility through DDA, DHR or Medicaid
- Make referral to CSC's appropriate program through Service Coordination for adults with developmental disabilities, through a DSS social worker for medically fragile, behaviorally challenging and autistic children, and through families and/or appropriate referral entity for qualified adults to CSC's adult medical day care program.
- CSC's ARD (Admission, Review and Discharge) Committee reviews each interview and referral.
- Invitation to the individual, family, and/or advocate to visit the requested program site.
- Final decision and placement
Special Considerations/Comments:
CSC is amenable to opening additional residential sites, and, where necessary, providing homes that will require handicap accessibility and modifications.
