Discuss How Your Agency Has Utilized The "Ask Me!" Survey Results.

The "Ask me" Survey Results are used as a bench mark for comparing the level and quality of care against providers in maryland as a whole. It provides a measure to assess SHSG level of service vs the standards in each domain of the " Ask Me" Survey Results.

The findings of these Survey Results guide SHSG's efforts in the designing of better individual plans that would result in self-fulfilled outcomes that would greatly enhance individual satisfaction.

The 2006 "Ask Me!" Survey Results is available for the general public

  5. ENCOURAGE INDIVIDUALS TO REGISTER AND PARTICIPATE IN SPORTS ORGANIZATIONS FOR THE DISABLED - eg special olympics

Steps Individuals Must Take to Apply for Services

  1. CALL OFFICE FOR ADMISSION PACKET
  2. COMPLETE AND RETURN PACKET
  3. ESTABLISH FUNDING ELIGIBILITY DDA OR OTHER SOURCES
  4. ARRANGE FOR A VISIT TO SHSG's FACILITY
  5. ARRANGE FOR INDIVIDUAL TO HAVE DINNER AND SPEND THE NIGHT IN ONE OF SHSG's FACILITIES
  6. WRITE SERVICE FUNDING PLAN (DDA) OR WRITE PAYMENT PLAN (NON-DDA)
  7. SIGN SERVICE CONTRACT WITH SHSG INC.
