To provide opportunities that allow adults with disabilities to enjoy increased self-determination in leading a more independent, valuable, and functioning role in society. These programs are professionally- managed and individually-designed to empower these adults to exercise higher levels of control over their lives.

Discuss How Your Agency Has Utilized The "Ask Me!" Survey Results.

The Center has modified its individual satisfaction tool to integrate the domains captured in the survey. The Center recognizes that quality of life, dignity, and personal choice and freedom are important to each individual that utilizes its programs and services. The Ask Me! survey has assisted the Center in reaffirming its definition in a way that is consistent with the Ask Me! survey.

Discuss your quality assurance practices, including any agency accreditations

Providence Center has a Program Oversight Committee made up of Board members, staff and individuals that meet monthly to discuss quality assurance practices. Each division within Providence Center has specific annual goals for which data is collected and reported to this Committee quarterly. The different divisions also record monthly reports to present to this committee which include highlights on each program, individual progress, assessments, interviews, hires, personnel trainings, volunteers, fire drill reports and water temperatures at all locations. All individuals complete an annual Individual Satisfaction survey that is used to determine quality programs and initiate possible new services. Employers and Businesses that PCI is actively is actively working with receive annual satisfaction surveys as well. Case Record reviews are done quarterly. A Standing Committee meet quarterly and reviews all incident reports and reportable incidents to ensure all processes were followed correctly as well as to ensure health and safety practices. Staff development through various trainings offered are encouraged by all.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

Providence has a Community Connections Committee with representatives from each program at PCI to discuss and network to meet the community wants/desires/needs of the folks in each program. This committee meets quarterly with a goal matching 10 folks a quarter. Providence Center also has a successful Supported Employment program with 95 individuals currently independently employed in Anne Arundel County. The Art Institute offers classes in performing and visual arts to 135 artists at Providence Center. Through various venues in the community the artists display their pieces, perform, make money, build self-esteem and truly shine. Providence Center also sponsors vacations, day trips and other educational experiences that are community based.

Steps Individuals Must Take to Apply For Services:

Interested parties can call the Admissions's Department and a Community Outreach Coordinator will be available to provide information about the agency and assist in identifying if an individual may be a candidate for Providence Center's Services and walk the applicant through the application process.
