Unique or Distinctive Qualities

The individuals and their family members are the very first involved in the initial planning process as well as annual Interdisciplinary Team process and informal meetings related to problems, issues or other changes in the individual's life that would require additional planning. St. Patrick Homes is very concerned with the importance of the individual, family and staff working together to develop services that best meet the individual's need.

St. Patrick Homes will examine the information from the "Ask Me" Survey to be used to develop a plan, to improve the service improve the service for the individuals and to address concerns.

Describe your quality assurance practices, including any agency accreditations

St. Patrick Homes has a Standing Committee form up of Board members, family members, individuals that the St. Patrick Homes served and staff who actively participate on Quality Assurance Committee that meet monthly or anytime an emergency occurs to review the protocol of the Agency for identifying, reporting, documenting, investigating and reviewing of incidents to ensure compliance with Administration procedures. The committee reviews the policies and procedures of St. Patrick Homes and implementation to ensure that they adequately protect the legal and human rights of the individual served. St. Patrick Homes gather sources of data that include individual satisfaction survey, investigation reports, incident reports to send to the families as well as to ensure health and safety practices. A variety of data sources will be reviewed to identify trends requiring focused quality improvement activities. Staff are regularly trained in compliance and quality issues
