Supported Employment, Day Habilitation Program, Individual Support Services, Vocational Day Program/Sheltered Workshop.

AIDS, Autism, Behavioral Problems, Chemical Dependency, Epilepsy/Seizure Disorder, Head Injury, Orthopedic Impairment, Specific Learning Disability, Speech/Language Impairment, Cerebral Palsy, Cystic Fibrosis, Mental Disorder, Mental Retardation, Multiple Sclerosis, Muscular Dystrophy, Spina Bifida, Spinal Cord Injury, Other Neurological Impairments and other undetermined disabilities.

Mission Statement

The Mission of The Outcomes Service is to enable adults with brain injury, cognitive impairments, neurological, and developmental disabilities to reach the highest levels of productivity, independence, and inclusion in the community.

Unique or Distinctive Qualities

The Outcomes Service utilizes a professional staff of case managers who not only provide employment services, but address the overall needs of the individual to function as independently as possible. Recognizing the need for more social and recreational activities, The Outcomes Service offers a monthly Consumer Support Group where members plan and choose their activities. Events have included barbeques, pizza and movie nights and trips to area attractions. Additional service may include practical applications of Speech/Language strategies and access to a Social Worker. Also, in partnership with Adventist Rehabilitation Hospital and The Outcomes Service, the Rockville Brain Injury Support Group meets twice monthly to allow survivors and their families to better cope with long range implications of their disabilities.

are part of the Quality Assurance Program that is done every 6 months at Outcomes. The results are shared with Outcomes staff, Outcomes Standing Committee, The Treatment and Learning Centers Management Team and Board of Trustees. Outcomes will make changes in programs as necessary based on the results.

Describe your quality assurance practices, including any agency accreditations.

For the past 10 years, The Outcomes Service has been accredited by CARF (Commission on Accreditation of Rehabilitation Facilities) in Comprehensive Vocational Evaluation, Employee Development Services, and Community Employment Services in Job Development, Job-Site Training, and Job Supports. These programs continually receive the highest accreditation possible (3 years). CARF accreditation is an important part of Outcomes Quality Assurance Practices. The Treatment and Learning Centers/Outcomes Program extensively evaluates all programs every 6 months and makes any recommended changes. The areas evaluated include program efficiency, effectiveness, health and safety, behavior management, and satisfaction of persons served. The results are shared with Treatment and Learning Centers Management Team, Board of Trustees, Standing Committee, and are available through The Outcomes Service Annual Report. Consumer satisfaction results are posted on the Treatment and Learning Centers web site at Web Address: www.ttlc.org.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

The Outcomes Service Staff see a world in which everyone has the opportunity to reach their highest potential. Outcomes case managers provide guidance to allow persons with disabilities to fully participate in the community by engaging in employment, maximizing independence and accessing community resources. Every person served by Outcomes is assigned a case manager who acts as an advocate and offers support in attaining the goals of each individual. Examples of this support and advocacy may include travel training, assistance with getting a driver's license, linkages to social and recreational activities, assistance with housing, and volunteer activities.
