Steps Individuals Must Take to Apply for Services

For DDA services: Complete a DDA application and mail it to the National Multiple Sclerosis Society, Maryland Chapter who will then forward it to DDA

For Chapter Services: Complete a client registration form and become a registered member with the chapter.

Special Considerations/Comments: Chapter can only serve individuals who are diagnosed with multiple sclerosis.

Funding Sources: Developmental Disablities Administration, other grants, fund raising events, donors.
