Mission Statement

Lifeline, LLC will provide out of home care to children with medical and /or developmental disabilities that can no longer be cared for by their biological families. The prgram is committed to working with area hospitals to provide residential, medical and social services for children no longer meeting medical necessity criteria for continued hospitalization. Care will be provided ina an individual home setting to include a full array of medical, social work and support services necessary to allow the children to develop into healthy adults.

Steps Individuals Must Take to Apply for services

  • Contact agency either directly or through Service Coordination
  • Meet with personnel to identify services desired
  • Complete application
  • Arrange meeting between individual and/or family
  • Visit one of the agency homes
  • If interested, secure funding and admission approval
