Mission Statement

Jubilee Association of Maryland provides opportunities and support for people with developmental disabilities to live in and enrich their community while fulfilling their personal, family, and spiritual needs. The Association bases its work on a belief in the dignity and worth of all people as well as their right to pursue hapiness as full, respected members of our society.

Jubilee Association is sponsored by the Hyattsville Mennonite Church and is guided by the teachings of the Christian Faith.

Jubilee Association has identified areas of concern from its Ask Me survey and set agency-wide goals to make improvements in those areas.

Describe your quality assurance practices, including any agency accreditations?

Jubilee is licensed by the Maryland Department of Health and Mental Hygiene on the state level and by the Montgomery County Department of Health and Human Services on the county level.

Jubilee has a strategic plan with goals and objectives that the Board of Directors regularly reviews. Jubilee also has a quality assurance plan which includes annual goals. In addition Jubilee participates in the Ask Me survey, a consumer satisfaction survey administered under the auspices of the Arc of Maryland, by people who have disabilities.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

Jubilee provides supports to people who live both in their own and in agency-owned housing. In either case, people choose where they want to live and whom they live with, whether the roommate is a friend, family, or paid support personnel. Jubilee strives to ensure that people live in warm and caring places where they enjoy spending time with people they like. "Home" is a place for relaxation and growth, where people can follow their own routines and learn skills and responsibility in a natural environment. At the same time, Jubilee continually works to guarantee that people's health and safety needs are met.

In line with self-determination principles, Jubilee designs its supports around the individual. Jubilee begins with a planning meeting that includes the person and those closest to him or her to determine who the person is and how the person would like to live. Jubilee examines likes and dislikes, wants, needs, and dreams and design supports to accommodate. Jubilee helps people develop their talents and introduce them to and help them explore new possibilities for leisure and other activities.

Because Jubilee believes that all people should have equal access to what the community offers, Jubilee help people develop the network of connections that weave them into the life of the community. Jubilee also encourages staff to develop community relationships both for themselves and for Jubilee because it can only connect others when itself is interconnected. Jubilee reaches out to people and resources in the general community to find opportunities for those it supports to work, learn, and play. As a faith-based organization, Jubilee considers it particularly important to help to help people connect, as they choose to participate in their own religious community. Jubilee continually looks for ways to support people to become active members in their community, including helping them take on leadership and civic roles.

Steps Individuals Must Take to Apply for Services

Any Individual, or their family or advocate, wishing to apply or to gain information concerning the availability of services should contact the Jubilee office and speak to the Admissions Coordinator or submit an application for services. Applications are available on the Jubilee web site, or they can be mailed to the individual.

Admissions decisions are made based on a variety of factors. The applicant and his/her family or advocate is encouraged to call or write periodically to maintain an up to date application file. Jubilee will regularly update its waiting list regarding continued interest, changes in the individual's situation and Jubilee's expansion plans.

Admission Materials Required
An application is only a document required to apply for services and to be placed on the Jubilee waiting list. A current medical report, psychological report, social history, dental report and Personal Futures Plan will be reviewed when the Admissions Committee is considering the individual. These materials will be used to assist Jubilee in better serving the applicant. Jubilee is interested in understanding the applicant's disbilities and limitations as well as the applicant's abilities and interests.

No matter where the person lives, Jubilee's services are designed around the specfic needs of the individual. Jubilee assesses each person it serves and develops an individualized plan on the individual's strengths, needs and preferences.
