Guidelines are set by the Federal Guidelines and the State of Maryland which must be met. Each hospice must abide by these regulations in order to be a hospice program.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

Steps Individuals Must Take to Apply for Services:

  1. Family or patient requesting to receive comfort care/hospice
  2. Have their physician certify that the patient is terminally ill (best judgement 6 months or less to live)
  3. Meet hospice appropriate guidelines clinically set fourth by Centers for Medicare CMS)
  4. Have/meet payment options i.e. insurance. See funding source
