The mission of Forward Visions is to actively and meaningfully engage individuals in actualizing their ability to assume their rights as citizens within the mainstream of society to greatest extent possible.
Forward Visions provides a variety of community living options for individuals with developmental disabilities. Forward Visions has one eight and one four person group home as well as nummerous two and three person alternative living units in its residential program. Its relatively small size allows the flexibility to meet the needs of individuals who have additional needs or diagnoses. Forward Visions works closely with family members and welcomes family support and input. Each individual it serves is encouraged to decorate hi/her bedroom in the style of their choice. The agency schedules and takes individuals on all doctor and therapy appointments as well as on activities of social or recreational nature. Individuals in CSLS and Support services receive individualized services based on their needs and preferences. Individuals in support services are encouraged to participate in the hiring process of their staff.
Forward Visions participated in the "Ask Me!" survey for the first time this year. The agency will use these survey results in conjunction with other data collected from its quality assurance reviews to further its goal of providing quality services to individuals that meet specific preferences and needs.

Forward Visions believes that true quality assurance should be practiced each day with each individual whom it serves. Its staff work with each client on a daily basis to assist them in making their own choices and in acknowledging, and supporting their personal preferences. The agency has a quality assurance plan that is implemented by a specfic committee. Each Individual receiving services is encouraged to complete an annual self satisfaction survey that is used in several ways. On an individual level it can be used to help each person gain more control and choice in their daily life. On an administrative level it is used to help improve the quality of services that are delivered agency wide. Forward Visions recently hired a staff person who will be working in the areas of quality assurance and staff training.
Forward Visions encourages and supports each individual in community activities of their choice. Staff assist individuals in choosing the activities they would like to participate in and support. Staff assist with scheduling, transportation and any additional assistance the individual may require.
The individual applying for services must be approved by the State of Maryland Developmental Disabilities Administration (DDA) for funding. The Agency accepts referrals from Service Coordinators, DDA, individual's and/or their family
Special Considerations/Comments: Forward Visions does not maintain a waiting list for services. Each Individual who applies for services is evaluated on an individual basis with special consideration given to the residential opening/s that are available and compatibility of the needs and preferences of all involved.
